Cervical History & Exam Documentation Form - Available for Download
The Cervical History & Exam Documentation Form was recently added to the Professional Resources section of the website. Focus tested by Every Woman Counts clinicians in summer 2011, the form is suitable for use in clinics that provide cervical cancer screening services. The Cervical History & Exam Documentation Form may be used in conjunction with the CDS-provided Clinical Breast Exam Results Documentation Form, also available from the Professional Resources section of the website. CBE Technique Video
The California Department of Public Health, Cancer Detection Section, has produced a third edition of the video Core Competencies of CBE. The video covers the nine key elements of a comprehensive clinical breast exam, as determined by a national workgroup of breast screening experts and published in Clinical Breast Examination: Practical Recommendations for Optimizing Performance and Reporting, CA Cancer J Clin 2004;54:327-344. Breast Cancer Diagnostic Algorithms - 4th Edition - Available for Download
The Breast Cancer Diagnostic Algorithms for Primary Care Providers has been updated by the CDS Breast Expert Workgroup (June 2011). The Algorithms were developed to aid primary care clinicians with the work-up of breast abnormalities and to promote the practice of routine risk assessment. Originally published in 1997, the 4th edition incorporates the latest research and guideline updates into a brief, user-friendly format.
Health care providers are encouraged to use the algorithms as an adjunct to clinical decision-making; they are not intended to replace clinical judgment with regard to individual cases.

Revised Breast Cancer History & Risk Assessment Form - Available for Download
The Breast Cancer History and Risk Assessment form has been revised (June 2011). This patient information form is suitable for use in any clinic providing screening services. The form can be used in conjunction with the Risk Assessment Table (see Breast Cancer Diagnostic Algorithms) to help identify women who may be at increased risk of developing breast cancer.

Revised CBE Documentation Form - Available for Download
The California Department of Public Health has revised the CBE Documentation Form, suitable for use in any clinic providing clinical breast exams. When completed with care and accuracy, it is a valuable tool for both assisting providers with the communication of key findings within and across specialties and for clinical risk management.
